How We Do It Differently
Most clinics make you wait six months for a tooth. We work to shorten that.
At most Sydney dental practices, replacing a single tooth with an implant is a multi-stage process spread over 6 to 12 months. Extract the tooth, then wait for healing. Place the implant, then wait for integration. Fit the crown. During much of that time, you walk around with a gap, a temporary denture, or a bridge you didn’t want.
We use a one-surgery concept wherever clinically possible. In suitable cases, Dr Cai places the implant at the same appointment as the extraction — and where bone quality and primary stability allow, fits a provisional tooth the same day. Not every case qualifies, but many do, and we’ll tell you honestly at your consultation whether yours is one of them.
Immediate placement and same-day provisionals aren’t suitable for every case. Active infection, insufficient bone, or poor primary stability may mean a staged approach is safer. We’ll always tell you which applies to your situation and why.
About Dr Michael Cai

Dr Michael Cai has placed and restored over 3,000 dental implants over more than 18 years of focused implant practice — placing him among the most experienced implant dentists in Australia by personal case volume. Across Pitt Street Dental Centre, the team has over 5,000 implants of combined experience.
Dr Cai completed the Master Clinician Program in Implant Dentistry at the University of California, Los Angeles, and additional training at the GIDE Institute. He holds a Bachelor of Dental Science with Honours from the University of Western Australia and is a member by examination of the Royal Australasian College of Dental Surgeons (MRACDS GDP). His research has been published in international dental journals.
Dr Cai is accredited at both Westmead Private Hospital and Strathfield Private Hospital, allowing complex procedures — and patients who prefer general anaesthesia — to be managed in a hospital theatre setting. He also trained as a dental technician before practising dentistry, giving him an unusually deep understanding of how implant restorations are designed and fabricated.

Hospital accreditation is granted only to clinicians who meet specific surgical credentialing standards. It’s a level of institutional accountability that most CBD dental clinics don’t carry

Risks & Recovery
Dental implants are a long-term option for replacing missing teeth, but they involve surgery and carry specific risks and recovery considerations.
Possible risks may include:
- Swelling, bruising or discomfort after surgery
- Minor bleeding during the initial healing period
- Infection at the implant site, though uncommon
- Risk of implant failure if integration with the bone does not occur
- Nerve injury or sinus involvement, depending on implant position
- Higher risk of failure if you smoke or vape
Recovery considerations:
- Initial healing generally takes one to two weeks
- Osseointegration, where the implant fuses with bone, can take several months
- A soft diet is often recommended during early healing
- Regular reviews and good oral hygiene are important for long-term success

Frequently Asked Questions
Will I walk out with a tooth on the same day?
In suitable cases, yes. Where bone quality, primary stability, and the clinical situation allow, Dr Cai can place a provisional temporary tooth at the same appointment as the implant. This is not possible or appropriate for every case.
Factors like bone loss, active infection, or the need for grafting can make immediate loading the wrong clinical call. We will tell you clearly at your consultation whether a same-day provisional is right for your case, and give you a separate fee for it if it is.
The fee says "from $6,000". What might the total be for my case?
The $6,000 covers implant surgery, abutment, crown and radiographs. If your tooth needs to be extracted, if you need a bone or gum graft, or if you want a same-day provisional tooth, those are quoted separately after we’ve assessed your specific situation. Most straightforward cases don’t need grafting. We will give you a written quote covering everything applicable to your case after your consultation. No surprises.
Is the $250 consultation refundable?
If you proceed with implant treatment at our clinic, the $250 consultation fee is credited back to your account, so in effect, the consultation costs you nothing if you go ahead. If you decide not to proceed, the $250 covers the time, clinical assessment, and 3D imaging.
Does the procedure hurt?
The procedure is performed under local anaesthetic. Most patients describe it as similar to a tooth extraction, some pressure and sensation, but not pain. Twilight sedation is available for patients who prefer to be unaware of the procedure, and general anaesthesia in hospital theatre for those who prefer it. Post-operatively, mild discomfort for 2 to 3 days is normal and well-managed with prescribed medication.
How long do Straumann implants last?
Straumann implants have published 10-year survival rates above 95% in healthy patients. Longevity depends on bone and gum health, oral hygiene, and regular maintenance. We’ll discuss realistic expectations for your specific case at consultation.
What about getting implants overseas for less?
Overseas clinics, particularly in popular dental tourism destinations, typically use generic unbranded implant components with limited clinical data. Long-term aftercare is also a major issue. Implant complications often emerge over years, and managing them from 8,000km away is impractical. If you have overseas work that needs managing, we can help. But for initial treatment, the price difference rarely reflects good value when lifetime costs are considered.
Can I use my superannuation to pay?
In some circumstances, yes. Early Release of Super on compassionate grounds may be available for serious dental conditions. We work with SuperCare and similar services. Eligibility is determined by the ATO. We’ll explain the process at consultation.
After-Care for Your Dental Implants
Dental implant placement involves a surgical procedure and it is normal to experience some discomfort and swelling afterward. You will be given specific aftercare instructions for your situation. The following are general guidelines that may apply across both phases of treatment:
